Please complete the NEW PATIENT PACKAGE in advance of your first visit.  Additionally, in order for us to provide you with the highest quality of care, we must receive a copy of your current medical records, neurology notes and a referral letter from your primary care physician or neurologist prior to your first appointment. We are sorry that we cannot make an appointment for a new patient prior to receiving these items.
